How are fossil fuels formed?

Fossil fuels are hydrocarbons. primarily coal fuel oil or natural gas, formed from the remains of dead plants and animal in order for a fossil fuels to form they need to be underground for millions of year.  I can't exactly remember how many millions exactly but fossil fuels are dead organism.
